Ad Submission Form

CLICK HERE for Flyer that was passed out at Parent Meeting. Use a printed out copy of that form to submit an ad with camera-ready art. Fill out the form below to submit a text-only ad.

Each Spirit Pack comes with $50 worth of ads for this year's Soccer Program Book. These ads can be 2 business card size ads, or 1 one-quarter page size ad. Additionally, ads of other sizes are available (please view the rates & sizes on the FLYER). All ads MUST be submitted by Friday, December 8, 2006.

If you would like to place a text only ad, please fill out the form below. Please note on the form whether it is one of your Spirit Pack ads, or if it is an additional ad that you are purchasing separately. If it's not one of the Spirit Pack ads, please send your payment to:

Ad Info:

You may purchase as many ads as you like. If you would like to submit art for ads, please print out the FLYER (click here) and send with your payment in time to meet the December 8 deadline.

Ads should be provided as camera-ready art, black and white only (except for the back cover, which can be a 2-color ad). 300dpi, with all images and fonts included; PDF format preferred, EPS, TIF and Photoshop images acceptable. Business Cards can be submitted in lieu of camera-ready artwork.

Text ads submitted using the form to the left will be designed to give it a professional look in the Program Book.
